What the request should include
- Exact product or service name, model, link, vendor, and estimated cost.
- The expected FES-UA category and any supporting provider information.
- A specific explanation of how it supports this student’s learning needs.
- Relevant evaluation, prescription, curriculum, or professional recommendation when the category calls for it.
Anonymized educational-benefit example
“Student A has difficulty maintaining a stable writing position and completing written assignments independently. The requested adjustable desk provides the height and positioning recommended for the student’s home learning setup. It will be used during daily reading and written-language instruction. The request is for one desk priced at $219 from Example Vendor.”
Replace every detail with the student’s actual need and the exact requested item. A generic statement such as “this will help with school” gives the reviewer less evidence.
After approval
Approval applies to the specific item or service reviewed. Keep the approval with the later invoice and proof of payment, purchase only what matches the approval, and still meet the normal reimbursement requirements. Approval does not guarantee provider eligibility, available funds, or final reimbursement.
